About Chambesy statement of 1993, does anyone know which churches did adopt it officially?

About Chambesy statement of 1993, does anyone know which churches did adopt it officially?

Not all of the "churches" adopted it officially, nor did any of those "churches" condemn the bishops that signed it, and almost every World Orthodox church sent a bishop to sign this document. Now they call this document "theological recommendations".
