Question: Shouldn't a hierarch's face be covered? St. John Maximovitch was covered. St. Philaret was covered. Vladika Vitaly was covered.

Not systematically. Traditionally, in Greece, he bishop is sat on a chair even dead and the face uncovered. But he can be in a coffin too, but with the face uncovered.
